Some Pride HS Summer Teams travel our of State. Are you willing to incur the additional costs of that travel? Ex. Team Vans, Hotel Room (4 players per room, and additions money to be sent with your son for food etc.*

Age Determined by age of player as of April 30th of current year.

When  setting rosters, Pride places players on teams based on a combination of maximum individual development, team need, and team competitiveness. 8U-14U players are willing to accept assignment to Select/AA, Elite/AAA, or Premier/Majors based on information provided on this form.

WAIVER OF CLAIM

By signing below I agree to allow my child to participate and I am stating that I understand there are certain inherent risks associated with participation in baseball practice, camaps, clinics, and games and hereby agree to assume the risk of injury from these activities and agree to hold harmless and waive any claim for damages against the Pride Baseball Academy, its hired coaches, directors, volunteers, as well as the owners of venues utilized for these activities. I further understand that in the event of a medical emergency, EMS will be called to render aid and that I will be financially responsible for any expenses involved.

Agreement to Participate

With my electronic signature below I certify that (1) I possess a sufficient degree of physical fitness and mental capacity to participate in baseball opractice and related activities, (2) I will communicare with the instructor/coach about any and all disconforts, ains, injuries, etc. and (3) I will indicate any health related conditions that might affect my ability to safely participate. I have read and answered all the questions above and have had all my questions answered. I know, understand, and appreciate the risks associated with baseball and other related activities and I am voluntarily participating in the activities. In doing so, I am assuming all the inherent risks of the activities.
